Getting Started with Your
Functional Nutrition and Herbal Therapy Program

A personalized nutrition plan, designed just for you can support your body in expressing complete functional wellness. All of this can be achieved without having to step foot into an office. Learn how to get started in only three steps:


Complementary 15-min Consult – Phone Appointment

The initial complementary phone appointment is about 15 minutes. The purpose of this phone call is to discuss your overall health and wellness goals as well as effectively communicate our program to you. Any questions you have about our services will be answered here and if you decide you would like to proceed, the next step is completing the online nutrition paperwork.

Online Nutrition Paperwork – E-mail

After the initial 15-min consult, the next step is to complete the online nutrition paperwork. There are two pieces of paperwork that need to be completed:

  1. Terms of Acceptance – a legal document describing the scope of wellness services that will be
    provided, a fillable pdf where you can type your name and email it back to us for our records.
  2. Systems Survey – paperwork to be completed online, so no need to print it out and send it in. As soon as you complete it, the data is sent to us directly. None of your paperwork or personal information is stored online to respect the privacy of your health information and HIPAA laws.

After we have received your completed paperwork, you will receive an e-mail invoice through PayPal* for payment. Once Society of Wellness, Inc. receives payment through the PayPal Invoice, we will schedule your next appointment, Visit 1: Functional Nutrition Assessment.

*Note: You do not have to have or create a PayPal account to complete the invoice, you may check our as a guest through their payment system. For any questions or help through this process, please contact us directly.


Visit 1: Functional Nutrition Assessment – Phone Appointment

Once we have received your online nutrition paperwork, we proceed to our Functional Nutrition Assessment, this is about 25-30 minutes. The purpose of this appointment is to gather more data about your current state of health, including any previous lab work for future visits. We will also be asking more specific questions relating to the paperwork that you have completed. Some of the topics of the questions will include the following, but not limited to:

  • Medications
  • Menstruation
  • Supplements
  • Typical Diet
  • Allergies
  • Exercise
  • Surgeries
  • Occupation / Work Schedule
  • Sleep Habits
  • Personal History of Illness
  • Bowel Movements
  • Family History of Ilness

Prior to this appointment, you may want to gather records and make notes about the above topics. For example, if you take medications, please have the names of each medication and dosage readily available for the ease and efficiency of our appointment. After the Functional Nutrition Assessment has been completed, we will e-mail your results and schedule our next appointment to discuss it in detail.


Visit 2: Report of Findings – Phone Appointment

Once you have received your nutrition results via e-mail, we will discuss the details on this phone consultation, about 25-30 minutes. The purpose of this phone appointment is to communicate your specifically designed nutrition program in full detail. Some of the topics we will be discussing the Systems Survey findings and any whole food and/or herbal supplementation recommendations.
Once we review your program, you will receive an additional PayPal Invoice for your supplement protocol, prices vary based on your specific plan as not everyone needs the same supplements. As soon as Society of Wellness, Inc. receives the PayPal Payment, your supplements and instructions will be drop shipped to your home address within two business days.

Receiving and Beginning your Nutrition Program – Email

Society of Wellness, Inc. will follow up with an email on the day you receive your program in the mail. The purpose of this email is to confirm that you received your package and ensure that you are ready to begin your program.

That’s it, three steps to a nutrition program designed uniquely for you to support your overall health and wellness goals. And it’s all done on your watch, no need to take time away from your busy schedule for office appointments.

Preparing for Future Appointments

Visit 3: Functional Blood Chemistry Analysis – Phone Appointment

About 3-5 days after your Report of Findings visit, we will schedule a separate appointment to discuss your Function Blood Chemistry Analysis (FBCA), about 25-30 minutes. You will need to send any copies of the history of your blood lab results via email or fax. Ideally, we should receive these copies during the initial stages of gathering paperwork for submission. The purpose of this appointment is to review and discuss your blood lab results in an organized FBCA Report, which will be emailed to you ahead of time.

Visit 4: Food Diary – Phone Appointment

About 10-14 days into your program, we will follow up with a Food Diary appointment, about 25-30 minutes. The purpose of this appointment is to provide support and answer any questions that may have arisen during the first week of your program as well as review the details of your food diary. Any food recommendations discussed during this appointment will also be e-mailed to you for your records.
